Mountain Bend Medical Associates (MBMA) is a dedicated family medicine clinic located in Dunlap, Tennessee, under the leadership of Dr. May. Serving a predominantly rural community, MBMA prioritizes personalized, high-quality healthcare. However, like many rural practices, they faced significant operational challenges that could not be effectively addressed by conventional, off-the-shelf Electronic Health Record (EHR) solutions.
The Challenge
Prior to collaborating with our team, MBMA encountered several critical issues. The clinic relied heavily on outdated workflows that required extensive manual documentation, resulting in operational inefficiencies and potential inaccuracies. They also experienced limited interoperability among their laboratory, billing, and prescribing systems, creating significant delays and errors. Furthermore, the absence of centralized patient communication and automated scheduling complicated administrative tasks and patient interactions. Additionally, MBMA faced growing pressure to comply with HIPAA and federal regulations without adequate technical support, placing them at risk of regulatory non-compliance.
Our Solution
To solve these challenges, our team implemented and customized OpenEMR as the core of MBMA’s health IT infrastructure. A significant portion of the project involved developing three major system integrations: PathGroup (labs), Waystar (billing), and NewCrop (e-prescribing). Each integration came with its own technical and logistical hurdles, but our team was able to tailor solutions that brought all three into a unified OpenEMR interface.
Custom PathGroup Integration
PathGroup does not offer a public-facing API, so we engineered a solution using secure file transfer (SFTP). Lab orders were automated through structured HL7 message generation and securely transmitted to PathGroup’s system. On the results side, we developed a custom module that continuously monitors the SFTP folder for incoming lab results, then parses and reconciles those results directly within OpenEMR. Each result is matched to the correct patient encounter and accompanied by both in-system and email alerts to ensure timely follow-up. This integration dramatically reduced the need for manual data entry and the risk of delayed test result reviews.
Waystar Billing Integration
Waystar’s system offers a partial API, which necessitated a hybrid approach for integration. We automated the creation and secure transfer of billing files using SFTP, while the available API endpoints were leveraged to monitor claim statuses and identify denials in real time. To make this data actionable for MBMA’s staff, we built a dashboard widget directly into OpenEMR that visualizes the billing pipeline and provides quick access to claims requiring attention. This has led to faster reimbursement cycles and fewer rejected claims, thanks to earlier detection and intervention.
NewCrop ePrescribing Integration
NewCrop’s fully API-based architecture provided a more straightforward integration path, though it still required close coordination with their implementation team. We worked directly with NewCrop to obtain credentials, navigate their testing sandbox, and ensure all compliance steps were met. The result is a seamless e-prescribing module within OpenEMR, allowing MBMA providers to write and transmit prescriptions electronically without leaving the EMR environment. This not only improved patient safety and convenience but also ensured the clinic met eRx compliance requirements.
